Cynthia Rae Jervis, 54, Green Bay, passed away on Sunday June 11, 2017. She was born October 3, 1962 to Ray and Rosemary (Taylor) Jacobs. She is survived by her parents, her siblings as well as her two children, Jayme and Matt. Cynthia was preceded in death by her two siblings, Ray and Cheryl.
